Please bring a physical copy of your insurance card to every visit.

All deductible, coinsurance, and past due balances are due at the time of service.

Office visits will be assessed an estimated charge for those with high deductible plans and coinsurances based on our contracted rates with your insurance carrier.

Self pay balances are due at time of service. We do offer prompt pay discounts.

Failure to provide 24 Hour notice on appointment cancellations will result in a $25 no show fee for office visits and a $50 no show fee for procedures.

We are currently not accepting any new Medicaid or Medicaid Managed Care patients.

The only Exchange Products AWM participates with are Excellus and MVP.

The only Essentials Product AWM participates with is Excellus.
